aLittlephotoshop.com has an excellent tutorial that can be used bother for masking and for cleaning up a high key photo. It involves dodging and burning the alpha channels using extremely large brushes set to highlight or shadows mode. Because the brushwork is so imprecise, the computer selects only the dark or light pixels.
While the hair isn't perfect, it is as close as most other methods I have seen, but much easier.
Deke McClelland has a more involved technique from Scott Kelby's blog that also masks hair.
